Thank you for choosing San Juan de Dios Educational Foundation, Inc. – College! We are here to assist in the processing of your ONLINE ENROLLMENT.

Go to the REGISTRAR’S OFFICE TAB of the San Juan de Dios Educational Foundation, Inc. – College website (sjdefi.edu.ph) to:

1. download the AFFIDAVIT OF UNDERTAKING to be filled-out by your parent/guardian. The AFFIDAVIT OF UNDERTAKING or “KATIBAYAN NG PAG-SANGAYON” is required in the absence of the enrollment documents which you cannot personally submit because of the current pandemic restrictions and the non-availability of these documents at the moment The filled-out AFFIDAVIT OF UNDERTAKING or “KATIBAYAN NG PAG-SANGAYON” must be uploaded when you register for online enrollment using the Enrollment Portal at sjdefi.edu.ph.
2. view the ONLINE ENROLLMENT PROCEDURE to guide you in processing your enrollment.
3. check the ENROLLMENT REQUIREMENTS for submission either PERSONALLY BY YOUR ADULT REPRESENTATIVE OR THRU COURIER (example: LBS, JRS, Lalamove, etc.) on or before Aug. 31, 2021.

In case you are not able to submit the enrollment requirements on or before August 31, 2021, original copy of notarized AFFIDAVIT OF UNDERTAKING shall be submitted to the Registrar’s Office personally or thru courier not later than September 6, 2021 for you to be allowed to continue attending the online classes.

Enrollment Requirements

To be submitted to the Registrar’s Office of San Juan de Dios Educational Foundation, Inc. - College PERSONALLY by an adult representative or thru COURIER (LBC, JRS, Lalamove, etc.) in a sealed envelope with the student’s complete name, grade or year level, strand or course on or before August 30, 2021.

Senior High School

1. Grade 10 Report Card (original)
2. PSA / NSO Birth Certificate (clear photocopy)
3. Certificate of Good Moral (original)
4. I.D. picture in white background (2 pcs)

College Freshman

1. Grade 12 Report Card (original)
2. PSA / NSO Birth Certificate (clear photocopy)
3. Certificate of Good Moral (original)
